WHAT IS TRANSITION?
Transition refers to the period of time from age 14 into young adulthood, when students prepare for and make the “transition” from high school to adult life. Transition is a process of preparation for and movement into adult life. For students with IEPs, transition services are provided by teachers, guidance counselors, AEA staff and community agency representatives, to support youth with disabilities during this critical time. For secondary students, the entire IEP process becomes focused on transition, beginning at age 14. Families play a critical role in supporting their sons and daughters during this important phase of their lives.
